Volunteer Agreement Each adult (18+ years old) in the home must complete this agreement in order to volunteer for Fetch Wisconsin Rescue. 1. Code of Conduct By volunteering with Fetch Wisconsin Rescue, Inc., (“Fetch”) I agree to uphold the following code of conduct: - I will maintain my integrity by keeping commitments and communicating promptly if I am unable to do so. - I understand that Fetch is made up of individuals with diverse temperaments, and I will respect the rights and feelings of fellow volunteers. - I will support the leadership, by-laws, policies, and operational structure of Fetch Wisconsin Rescue. - When I disagree with another volunteer, I will use established methods to lodge complaints and work towards resolving issues internally. - I will conduct myself with dignity and professionalism at all Fetch Wisconsin Rescue events. - I will work cooperatively with other volunteers to improve the welfare of animals. - I will represent a united front in public and address internal issues through the appropriate channels. * By checking this box, I agree to abide by and enforce this Code of Conduct and understand that violations may result in actions determined by the Board of Directors. 2. Care of Fetch Dogs By volunteering to care for a dog with Fetch Wisconsin Rescue, Inc. ("Fetch"), I acknowledge and agree to the following: - I will provide adequate food, water, shelter, and exercise for the foster dog in my care. - I agree to keep the foster dog as a house pet, not to leave it outdoors for extended periods of time, and to ensure its safety at all times. - I will follow all instructions provided by Fetch regarding the care, feeding, and medical treatment of the foster dog. - I will notify Fetch promptly if any issues arise with the health or behavior of the foster dog. In the case of an emergency, I will inform Fetch as soon as possible. - I understand that Fetch Wisconsin Rescue will provide for necessary veterinary care for the foster dog, including vaccinations and treatments. - I will contact Fetch before taking the foster dog to the veterinarian, even if it is an emergency. - I will notify Fetch in advance if I am unable to continue caring for the dog or if there are changes in my ability to care for the dog. - I understand that the foster dog remains the property of Fetch Wisconsin Rescue and that I do not have ownership rights over the foster dog. - I will not transfer ownership or possession of the foster dog to any third party without prior approval from Fetch. - I agree that Fetch retains the sole authority to approve the adoption of the foster dog to a permanent home. * By checking this box, I agree to abide by these terms and understand that violations may result in actions determined by the Board of Directors. 3. Acknowledgment of Risks, LIABILITY RELEASE and Indemnity Agreement: By checking the boxes below, I acknowledge and agree to the following: * I recognize that dogs can be unpredictable and potentially dangerous animals and that I may be injured or killed, or my property may be damaged, while being in the proximity of dogs in connection with my volunteer work for Fetch. * I understand that the risks of my volunteer work with Fetch include but are not limited to being bitten, tripped, or knocked down by a dog, or encountering a dog that behaves aggressively without warning or provocation. * With full knowledge of the risks involved, and in consideration for being allowed to be a volunteer for Fetch, I accept and assume full responsibility for any and all loss, injury, death, property damage, or any other harm that may occur while volunteering for Fetch Wisconsin Rescue. * I release, discharge, hold harmless, and otherwise fully indemnify Fetch, its directors, officers, agents, employees, and volunteers (“released parties”) from any claims or causes of action based on or arising from any such loss, injury, death, property damage, or any other harm I suffer in connection with my volunteer work for Fetch, even if such loss, injury, death, property damage, or harm arises from the NEGLIGENCE of the released parties. Nothing in this release waives any claims I may have against the released parties based on the reckless or intentional wrongful acts of the released parties. * I UNDERSTAND THAT THIS IS AN IMPORTANT LEGAL DOCUMENT, AND BY ITS TERMS I AM WAVING IMPORTANT LEGAL RIGHTS IN CERTAIN INSTANCES, INCLUDING THE RIGHT TO SUE. 4: Optional Alternative * By checking this box, I understand that I have the option to opt out of signing this Hold Harmless Agreement by making a one-time payment of an agreed upon sum of money with Fetch Wisconsin Rescue, Inc. This payment would serve as an alternative to signing the Hold Harmless Agreement and will absolve the Volunteer of the obligations and risks outlined therein. I understand that I have the option to contact Fetch Wisconsin Rescue to negotiate this, and elect not to. 5: Confidentiality * By checking this box, I agree to maintain the confidentiality of all sensitive information I may encounter during the course of my volunteer work. This includes, but is not limited to, personal and medical details about animals, donor and adopter information, financial records, and internal organizational matters. I agree not to disclose or share any such information with unauthorized individuals, either during or after my volunteer engagement, except as required by law. I also agree to take reasonable steps to safeguard this information and to return or destroy any confidential documents upon termination of my volunteer relationship with the organization. A breach of confidentiality may result in the termination of my volunteer position and possible legal action. 6: Agreement to Terms * By checking this box, I confirm that I have read and understand the terms and accept this agreement. As a volunteer of Fetch Wisconsin Rescue, I agree to abide by the policies and procedures. I understand that I will be volunteering at my own risk and that the rescue, its Board of Directors, other volunteers, employees and affiliates, cannot assume any responsibility or any liability for any accident, injury or health problem which may arise from any volunteer work I perform for Fetch. I agree that all the work I do is on a volunteer basis and I am not eligible to receive any monetary payment or reward. Submission of this agreement does not guarantee approval as a volunteer.
